On Aug 2nd, sports physicals will be given by appointment at the CVCH Specialty Clinic located in the Cobre Valley Medical Plaza in front of the hospital in suite 102. The physical is $32 & is payable at the time of the service by either check or cash. You'll need to bring the sports physical form from the school. The parent/guardian must fill out the front side of the form & sign it. If you have any questions call the Specialty Clinic at 425-3246 or 425-3247.
